Below are forms that are required for different services of AWCM.

• To become a volunteer of AWCM, create a TV or Podcast show, and/or assist AWCM staff; you must fill out the Membership Application.
  
  
• Once you have become a member of AWCM and you wish to create a program for TV or Podcast; you must fill out a Project Application.
  
  
• After you complete a project application and would like to request air time; you must fill out a Request for Cablecast.
